I made something similar to this for my daughter for her first year. I make a single layout page to put a couple pictures and then the 2nd page of the layout is going to be a calendar for that month. I write down all the important dates for that month : doctor appts, weight, height, 1st tooth ect., trips.

Hi Dana - I'll have to see what paper company it's from but I had picked it up from my local scrapbook store. If I find the paper company I'll post it for you
